Applicants must read and acknowlege the Volunteer Agreement before submitting a General Application. You may either fill out and submit the online application below or print out and return the .pdf file to GFMMF, POB 627, Glens Falls, NY 12801. We do not require a deposit until you have been accepted and we will contact you on receipt of your application.
The general application contains information about your medical history and must be completed before your application can be considered. Before you leave for Guatemala, we will need a copy of your passport and a copy of your professional licence if applicable. Payment of the $750 ($650 for students) may be made in one or two installments but we will need your full payment approximately one month before your departure. Your fee includes food, lodging, in-country transportation, and evacuation insurance unless you are over the age of 79. If you are over that age, you will be responsible for purchasing your own insurance. Your fee covers medications and supplies for the clinic too! Your trip fee may be paid either online with a credit card or by sending a check to GFMMF, POB 627, Glens Falls, NY 12801. Please keep in mind that the mission is charged 2% of your credit card payment to cover fees.
Mission team leaders, who are also volunteers, will contact you directly once you are accepted. Often, pre-trip meetings are scheduled in the Glens Falls area to meet your trip members, pick up medication and other supplies to bring with you, and learn more about what to expect. Arrangements are made for those of us who live too far away to attend.
